What If Medication Or Lifestyle Changes Don’t Help

Your provider may request more testing if lifestyle changes and medication did not help. These tests include a pH test to check for acid in your esophagus or an endoscopy. This test uses a small tube with a camera and light to see inside of your digestive tract and spot any issues.

Note The Time Your Heartburn Symptoms Occur

Think, too, about when the heartburn occurs. Galier says if it happens after a big meal, and it’s just the burning in the chest, with no other symptoms, itâs more than likely heartburn or indigestion. But if you have any doubt, it’s wise to ask your doctor for an evaluation.

“If you have chest pain after a meal, it’s more likely to be reflux,” agrees Glenn Eisen, MD, MPH, professor of medicine and director of endoscopy at the Oregon Health & Science University in Portland. But that’s not a perfect test, he says. “It could be cardiac.”

Phil Katz, MD, says that knowing what “classic” heartburn is like may help. Katz is president-elect of the American College of Gastroenterology and clinical professor of medicine at Jefferson Medical College in Philadelphia.

âThe classic heartburn symptom,â he tells WebMD, âis burning that starts at the upper stomach or lower breastbone and progresses upward and occurs after a meal or when bending over.” He adds, “It’s rapidly relieved by an antacid within a few minutes.â

Dont Eat Right Before Bed

This may be a bit more difficult if you go to bed early or if you attend a late-night party, but do your best to allow two or three hours of upright time to digest your meal before you lie down for a restful nights sleep. Even if you dont overeat before bed, gravity can work against you while you try to rest, placing pressure up against the LES – especially for stomach sleepers.

Tips To Keep Heartburn On The Backburner

To prevent occasional bouts of heartburn, try taking the following five steps:

1. Avoid eating within three hours of the time you go to bed.

2. Take any acid suppressant medication, such as a proton pump inhibitor, first thing in the morning. “Because of the way proton pump inhibitor-type medications activate, it’s not helpful to take these medications at night on an empty stomach,” says Brown.

3. Sleep on a slight incline, with your head elevated, and/or sleep on your left side. If you have chronic heartburn, falling asleep in the wrong position enables acid to sneak into the esophagus. Keeping your head raised slightly keeps that from happening. Studies have also found that while sleeping on the right side actually aggravates heartburn, flipping over to your left side is likely to calm it, although the reason why is unknown.

4. Avoid foods that may cause acid reflux, such as peppermint, coffee and chocolate. “Interestingly, spicy foods haven’t been shown to cause heartburn,” says Brown. “And there’s some evidence that spicy foods may actually protect the body from ulcers.”

5. If you smoke or use other tobacco products, quit. Not only does smoking relax the lower esophageal sphincter, allowing stomach acid to move up into the esophagus where it doesn’t belong, but it also can slow the production of saliva, which protects against acid in the esophagus.

What Is Acid Reflux

When you swallow food, it passes from your throat, through your esophagus, and then through an opening in your stomach.

This opening usually closes once the food passes through. If it doesn’t, acid from your stomach may enter through the opening and travel the wrong wayback into your esophagus and to your throat. This is called acid reflux. The opening can occur sporadically after a meal leading to multiple reflux events.

Food And Lifestyle Habits Can Cause Stomach Acid To Flow Back Up

Heartburn occurs when the contents of your stomach flow up into your esophagus.

A ring of muscle at the end of your esophagus where it joins your stomach known as the lower esophageal sphincter normally prevents your stomach contents from flowing back up into your esophagus by tightening after you swallow food.

But sometimes this muscle doesn’t tighten completely, and the entrance to your stomach doesn’t fully close. When this happens, stomach acid can enter your esophagus and cause heartburn.

There is a wide range of potentially problematic foods and other factors that can contribute to heartburn.

Galier says if you are reaching for antacids more days than not, you should see your doctor.

Eisen considers heartburn “occasional” if it occurs once a week or less. He says itâs time to seek medical help if you have had heartburn more than once a week for six months or longer more and are not getting better.

Whats Behind The Heartburn Claims

Your body also has its own pH levels, says Evan Dellon, MD, a gastroenterologist and associate professor of medicine at the University of North Carolina School of Medicine. For example, pH tends to be neutral in the blood, lower in the stomach, and higher in the small bowel, he explains.

The alkaline water industry claims that problems can arise when the body becomes acidic, and that their products can neutralize pH and promote a better balance.

But our bodies dont actually need help in this area, says Dr. Dellon: We have an incredibly sophisticated machinery to keep the pH levels where they should be throughout the body, and theres not a lot that you can do, eat, or drink to change that.

One 2012 study done in a lab found that water with a pH of 8.8 did seem to inactivate pepsin, an enzyme related to the production of hydrochloric acid . The water also had a buffering effect against hydrochloric acid itself. These in vitro data suggest that alkaline water may be a useful, risk-free adjunctive treatment for reflux disease, the study authors wrote, although they also stated that more research was needed, including studies in humans.

Dr. Dellon agrees: We really have no evidence that this works in real life scenarios, he says. Meanwhile, we do have many other reflux treatments that are well studied and pretty effective for people.

Why Is Gerd Worse After Going To Bed

There are several explanations for why GERD is commonly worse at night after going to bed:

  • When lying down, gravity no longer helps keep stomach acid down, making it easier for reflux to occur.
  • Saliva can help neutralize stomach acid, but production of saliva is reduced during deeper stages of sleep.

A combination of these effects can facilitate the leakage of stomach acid into the esophagus and allow the acid to remain in place for longer, potentially causing more severe GERD symptoms, including those that can disturb sleep. The problem may be even greater if a person goes to bed soon after eating and/or eats foods that trigger GERD.
